I decided to try making custom IEM tips. I had custom hearing protectors made a couple of years ago, so I used them to make a mold from silicone. Then I made copies of the originals with epoxy, drilled them out for sound to travel through, with the IEM side a bit oversize. Next I fitted them to the IEMs with hot glue, sticking them in my ears while the glue sets so that they point in the correct direction. Finally I finished them with a blue sharpie some UV cure clearcoat, since I don't think epoxy is great in terms of skin safety.

The result is a tip that always seals perfectly, while being more comfortable.
