Has anyone done either recently, and can outline the process?

Where do I get the forms?
Which office to even go to?
Are the eye tests and/or photos done in house, or must I arrive with them?
What else must I take with?
Where do I book a slot?
What's the cost?
How does one pay?
How much of a day can I expect to spend?
How many visits are needed?
Where do I even start?!!
