The diyAudio.com rules look like a perfect base to start. I'll add that to the existing ones and make a sticky post as suggested.

Another thing I thought of, with regard new 'members' who come on and obviously and deliberately start flame wars - most forums have either a rule/or request: "Don't Feed The Troll."

In other words, ignore them, let them get bored, and they'll go away soon enough.
Don't judge me for being quiet - no one plans a murder aloud.

I've also seen at DIYA that the first 5 posts of a new member gets moderated, ie. approved before being posted. Dunno if that's too extreme, seems to work well though

On other forums in which I am involved, we simply request the following credentials:

Real Name (Surname is optional)
Cell No. (if available - But this is not viewable by anyone else other than the Mods)
Town (Useful for like-minded enthusiasts. This is viewable by all under User's Profile.

We do not moderate initial posts per se, but rather watch out for transgressions of the forum's TOS. It works, the forum is strong and in-fighting simply does not happen. Email addresses are not required, a member may be contacted via the forum or PM if necessary.

Let the community do the authentication. Meet the people and do a report on the forum. Who wants to scan an id when you can braai.
